Any preterm baby should be followed till 2 years of age there are certain things which needs to be screened during this 2 year followup. First of all this eye screening should be done at regular interval by that I mean we have to screen retinopathy of prematurity. And this has to be followed up till 48 to 52 weeks of post menstral age. So you will have at least three to four more visits even after discharge just for screening of eyes. Hearing screening and if there is any hearing loss this has to be diagnosed before 3 months of corrected age. Third thing we have to do some metabolic profiling like thyroid profile this has to be repeated may be once or twice even after discharge because these preterm babies may have delayed hypothyroxinemia of prematurity and this gets missed if we do not repeat it 2 weeks and then again at 1 month of life. Then fourth thing is metabolic bone profiling. We need to look out that their bone are growing well their calcium and phosphorus deposits are going well. So we initiate this bone profiling at around 4 weeks of age and then after that we repeat it fortnightly till the paramters starts improving. Then we have a neurodevelopmental screening program every 3 monthly we have to assess the baby how they are developing all the Milestones are being achieved or not so in the first year every 3 monthly and in the second year even six monthly we do and during these session we have to assess the growth as well so that we know that our babies thriving well or not. Second question was that how to prevent infection in these pretrem population while we are at home. First of all what we can do is in these babies is promote exclusive breastfeeding and if at all you are using formula discourage the use of bottle feeding and third thing is that hand hygiene should be followed very religiously and you should avoid visitors which is the next question because this will always allow your baby to be exposed to viruses environment and then I would like to add that vaccination should be done at a regular interval. There's a myth related to the vaccination in preterm because we feel that our baby is quite weak we shouldn't start vaccination. So early because they will have a lot of pain but on the contrary because your baby is weak they are going to need the vaccination all the more.So you should not just go on their weight and you should get your baby vaccinated as per chronological age as per advice of the doctor.
