Notes are the foundation where learning happens for nurse practitioner students and new grads. When you sit down to align your physical and chart data to come up with a diagnosis and a plan to treat this patient that's when the magic hits. This is the point when a student looks at me and says, I don't know what to write. That's ok we all remember that feeling. Particularly when it comes to liver failure. There's just soooo much to address. That's why I chose to do some teaching about this particular diagnosis. Just break it down and catalog your thoughts/plan so the next time you see it you won't feel as overwhelmed.
