Sphenopalatine Block Demonstrated

Shenopalatine blocks using 2% lidocaine is a safe and effective way to abort a migraine episode, and can be used to interrupt chronic migraine activity. Contrary to popular understanding these blocks require no special knowledge of anatomy, tools for visualization of the anterior nose or the ganglion in the back of the nose, or devices. All that is needed is a 1 1/4 inch angiocath and a 3cc syringe to apply the 2% lidocaine. The technique and relevant anatomy are demonstrated. This treatment works because of its effect on the sensory branches of V2 and on the parasympathetic innervation of the intracranial space. While effective, direct injection of the ganglion with a needle is not necessary, making this a safe bedside, office and ER procedure.
