Form 1099DIV is filed to report dividends and other distributions. If you're an investor in a Regulated Investment Company (RIC), your Form 1099DIV may have other tax items reported.

Exempt interest dividends & private activity bonds are reported in Boxes 12 & 13, respectively. Exempt interest dividends represent the portion of a dividend distribution that is characterized as taxexempt interest income under IRC Section 103(a).
