This video explains the events that led to a forklift operator being crushed and killed when a forklift moved while unattended. This incident investigation slide show uses animation and photos taken from the incident site. No graphic imagery is included.

It describes the dangers of exiting a cab when a forklift’s load is raised and the hazards of overloading the rated capacity of the forklift. It details the risks forklift operators should be aware of when operating mobile machinery as well as the importance of lowering the forks of a forklift before exiting and keeping out of the crush zone at all times.
