In hot conditions provide:
Water ( a horse can drink over 25 litres of water a day, more in hot conditions )
Shade trees/cover
Ventilation breeze/fans
avoid work or travel during the hottest parts of the day

remember older horses or those with preexisting medical conditions can be more susceptible to heat stress prevention is better than cure
heat stress can present with raised temperature, pulse, respiratory rate and irritability
