Cruelty Squad is a game about suffering: Yours, your target's, and that of everyone living in the sewage world. Cruelty Squad's gameplay has been described to be a sadness laced version of Rainbow Six or Hitman but I prefer to think of it like your own personal cyberpunk assassination game.
